Im Mittelpunkt unserer Arbeit stehen klinisch relevante Aspekte von Infektionskrankheiten in den Tropen sowie deren reisemedizinische Bedeutung. Gemeinsam mit unseren afrikanischen Partnern engagieren wir uns in klinischen Forschungsprojekten um Behandlungs- und Präventionsmöglichkeiten nachhaltig zu verbessern.

Laufende Projekte

LADEX – Dexamethason in der Behandlung von Lassafieber

Lassafieber zählt zu den viralen hämorrhagischen Fiebern mit häufig schwerem bis tödlichem Krankheitsverlauf. Die Erkrankung ist in einem großen Teil von Westafrika endemisch.   Hauptübertragungsweg ist der Kontakt mit dem natürlichen Wirt des Virus, der Vielzitzenmaus und deren Ausscheidungen, das Virus kann jedoch auch von Mensch zu Mensch übertragen werden. Eine Impfung gegen die Erkrankung sind bisher nicht verfügbar, und die Behandlungsmöglichkeiten beschränken sich auf unterstützende Maßnahmen sowie das Medikament Ribavirin.

Dexamethason ist ein Kortikosteroid, das Gewebsentzündung unterdrücken kann und wird deshalb routinemäßig für eine Vielzahl von Erkrankungen verwendet. Es gibt Hinweise darauf, dass Dexamethason auch bei schwerem Lassafieber vielversprechend sein könnte.

Am Irrua Specialist Teaching Hospital (ISTH) in Nigeria, eines der größten Behandlungszentren für Lassafieber in Westafrika, wurde Dexamethason bereits erfolgreich in der klinischen Praxis eingesetzt, um Doppelinfektionen mit dem Lassa-Virus und SARS-CoV-2 zu behandeln. Die Kombination aus Ribavirin und Dexamethason wurde zuvor für die Behandlung von schwerem Krim-Kongo-hämorrhagischem Fieber getestet.

Die klinische Studie LADEX (Lassafieber-Zusatzbehandlung mit Dexamethason) soll erstmals eine Kombination aus dem Standard-of-care (Ribavirin) und Dexamethason bei Patienten mit moderatem oder schwerem Lassafieber evaluieren.

 

LADEX ist eine prospektive, offene, randomisierte, kontrollierte Phase-II-Klinikstudie mit zwei Gruppen:

a.  Antivirale standard-of-care Therapie mit Ribavirin

b. Antivirale standard-of-care Therapie Ribavirin + Dexamethason

 

42 Patienten mit moderatem oder schwerem Lassafieber sollen bis 2026 eingeschlossen werden. Der Studienbeginn ist für Ende 2023 geplant. Das BNITM ist der Sponsor der Studie, die Rekrutierung der Studienteilnehmer erfolgt am ISTH.

Das Ziel der Studie ist es, das Outcome von mittel- bis schwer erkrankten Lassafieberpatienten zu verbessern.


Klinische Evaluierung von Antimalaria-Tripletherapie mit Atovaquon-Proguanil zur Behandlung von Malaria in afrikanischen Kindern (ASAAP)

Bei ASAAP handelt es sich um eine klinische Studie zur Bewertung der Wirksamkeit, Sicherheit, Verträglichkeit und Pharmakokinetik eines neuen Malaria-Kombinationspräparats, mit einer Laufzeit von 48 Monaten. Der Schwerpunkt liegt auf dessen erfolgreicher Anwendung in einer der größten und anfälligsten Bevölkerungsgruppen Afrikas - Kinder von 0-5 Jahren. Das übergeordnete Ziel ist die Entwicklung neuer Antimalariabehandlungen durch die Kombination bestehender, wirksamer Arzneimittel zur Behandlung von Malaria bei afrikanischen Kindern durch die Förderung der afrikanischen und europäischen Forschungszusammenarbeit und die Stärkung der Fähigkeit afrikanischer Institutionen zur Durchführung klinischer Forschung.

ASAAP ist Teil des von der Europäischen Union (EU) geförderten European and Developing Countries Clinical Trials Partnership 2 (EDCTP 2) Programms und wird vom Bundesministerium für Bildung und Forschung (BMBF) und dem Nationalen Institut für Gesundheit und medizinische Forschung in Frankreich (INSERM) unterstützt.

Das Projekt wird von Dr. Oumou Maiga-Ascofaré vom BNITM in Hamburg, Deutschland und dem KCCR in Kumasi, Ghana geleitet, unter der Autorität der Kwame Nkrumah University of Science and Technology (KNUST) und in Zusammenarbeit mit:

MAMAH

Eine Malaria-Infektion während der Schwangerschaft wirkt sich erheblich auf die Gesundheit von Müttern und Neugeborenen aus, insbesondere bei HIV-infizierten Frauen. In Afrika werden jährlich mindestens eine Million Schwangerschaften mit Malaria und HIV co-infiziert. Die Interaktion zwischen den beiden Infektionen ist in der Schwangerschaft besonders schädlich, was zu einem erhöhten Risiko für Malaria und HIV-Viruslasten führt, was die Häufigkeit der Übertragung von HIV von der Mutter auf ihr Kind (MTCTHIV) erhöhen kann.

Eine intermittierende vorbeugende Behandlung in der Schwangerschaft (IPTp) mit Sulfadoxin-Pyrimethamin (SP) wird zur Vorbeugung von Malaria bei HIV-nicht-infizierten Frauen empfohlen. Bei HIV-infizierten Patienten mit Cotrimoxazol-Prophylaxe (CTXp) ist sie wegen möglicher Nebenwirkungen kontraindiziert. Eine kürzlich durch die EDCTP finanzierte Studie zeigte, dass zwar ein wirksames Antimalariamittel (Mefloquin) zu CTXp und lang anhaltende mit Insektiziden behandelte Netze (LLITNs) bei HIV-infizierten schwangeren Frauen die Malariaprävention und die Gesundheit von Müttern verbessert, jedoch nicht gut vertragen wurde und die HIV-Viruslast bei der Entbindung und das Risiko für MTCT-HIV erhöhte. Dies unterstreicht die Notwendigkeit alternative Arzneimittel mit einem besseren Verträglichkeits- und Sicherheitsprofil zu finden, um Malaria in dieser anfälligen Gruppe zu verhindern und die pharmakologischen Wechselwirkungen zwischen Arzneimitteln gegen Malaria und Antiretroviralen (ARV) weiter zu erforschen.

Dihydroartemisinin-Piperaquin (DHA-PPQ) verbessert nachweislich den Malariaschutz bei Frauen ohne HIV-Infektion und stellt den vielversprechendsten Kandidaten für IPTp bei HIV-infizierten schwangeren Frauen dar. Es gibt jedoch nur begrenzte Informationen zur Pharmakokinetik von DHA-PPQ bei gleichzeitiger Anwendung von ARV-Medikamenten und CTX.

Zielsetzung:

Eine randomisierte, placebokontrollierte Doppelblindstudie zur Bewertung der Sicherheit und Wirksamkeit von DHA-PPQ für IPTp bei HIV-infizierten schwangeren Frauen, die CTXp- und ARV-Medikamente erhalten und LLITNs verwenden, wird in Gabun und Mosambik durchgeführt, wo Malaria- und HIV-Infektionen moderat bis stark verbreitet sind. Die Möglichkeit einer pharmakokinetischen Interaktion zwischen DHA-PPQ- und ARV-Medikamenten wird untersucht. Frauen erhalten eine ARV-Therapie gemäß den nationalen Richtlinien, und ihre Kinder werden bis zu einem Jahr beobachtet, um die Auswirkungen von DHA-PPQ auf MTCT-HIV zu bewerten.

Das fünf Jahre dauernde Projekt wird von der EDCTP finanziert und von ISGlobal (Barcelona, ​​Spanien) in Zusammenarbeit mit dem Centre de Recherches Médicales de Lambaréné (CERMEL, Gabun), dem Gesundheitsforschungszentrum Manhiça/Fundaçao Manhiça (CISM/FM, Mozambique), der Medizinischen Universität Wien (MedUni Wien, Österreich), dem Bernhard-Nocht-Institut für Tropenmedizin (BNITM, Deutschland) und der Eberhard-Karls-Universität Tübingen (EKUT, Deutschland) koordiniert. Diese Studie wird schlüssige Beweise liefern, um die Belastung der beiden wichtigsten poverty-related diseases (PRDs) bei HIV-infizierten schwangeren Frauen zu reduzieren, die von Malaria bedroht sind.

Abgeschlossene Projekte


Etablierung klinischer Studienorte und Durchführung von Medikamentenstudien zur Verbesserung der Behandlung von Lassa-Fieber in endemischen Regionen

Die meisten afrikanischen Länder sind nicht in der Lage, Erreger mit epidemischem Potential sicher zu diagnostizieren und auf Ausbrüche adäquat zu reagieren. Das weitgehende Fehlen dieser Kapazitäten hat zur verzögerten Antwort auf den Ebola-Virus Ausbruch in West-Afrika beigetragen. Dazu kommt das Fehlen von medizinischen Gegenmaßnahmen für Erkrankungen mit epidemischem Potential, die ihren Ursprung in Afrika haben. Zu diesen Erkrankungen zählt ebenfalls Lassa-Fieber, das ähnlich wie die Ebola-Viruserkrankung ein sogenanntes virales hämorrhagisches Fieber ist. Es ist in weiten Teilen West-Afrikas endemisch. Jährlich werden mehrere hunderttausend Infektionen geschätzt, die Sterblichkeit im Krankenhaus liegt bei 30%. Eine Impfung oder sichere Behandlung gibt es nicht. Aus diesen Gründen wurde Lassa-Fieber von der WHO auf die Liste der Erkrankungen mit dringendem Forschungs- und Entwicklungsbedarf gesetzt.

Im Rahmen dieses Projekts werden in Nigeria Kapazitäten zur schnellen Eindämmung von Lassa-Fieber Ausbrüchen aufgebaut und medizinische Gegenmaßnahmen bei Lassa-Fieber Erkrankungen entwickelt. Dazu ist es erforderlich, bestehende therapeutische Optionen (Ribavirin) zu überprüfen, da Zweifel an der Wirksamkeit des Medikaments bestehen, und gleichzeitig neue Medikamente (z.B. Favipiravir) am Menschen zu prüfen. Dazu müssen entsprechende Kapazitäten für klinische Studien vor Ort aufgebaut werden und erste Studien mit den beiden Medikamenten initiiert werden.

Gemeinsam mit dem Irrua Specialist Teaching Hospital (ISTH) in Edo State werden Kapazitäten in Nigeria aufgebaut und Medikamentenstudien zum Aufbau der lokalen Expertise initiiert:

  • PAIRR: Eine prospektive Beobachtungsstudie zur Evaluierung pharmakokinetischer Eigenschaften der routinemäßigen Ribavirin-Behandlung von Lassa-Fieber-Patienten am ISTH.
  • SAFARI: Eine randomisierte, kontrollierte Phase II Studie zur Prüfung der Pharmakokinetik, Verträglichkeit und Sicherheit des Arzneimittelkandidaten Favipiravir alleine und in Kombination mit Ribavirin zur Behandlung von Lassa-Fieber am ISTH und am Federal Medical Centre Owo (FMCO).

Das Projekt SAFARI wird in Zusammenarbeit mit dem FMCO in Ondo State, Nigeria, dem Institut National de la Santé et de la Recherche Médicale (Inserm), der Université Bordeaux und der Alliance for International Medical Action (ALIMA) durchgeführt.

TROPICSAFE - Medizinischer Transport von Patienten mit tropischen Erkrankungen unter sicheren Umständen

Der Patiententransport ist eine gefährliche Aufgabe, wenn Fachkräfte mit ansteckenden Krankheiten zu kämpfen haben. Heutzutage gibt es mehrere Protokolle zur Behandlung von Tropenkrankheiten, die Angehörigen der Gesundheitsberufe fordern jedoch spezifische Richtlinien und praktischeres Schulungsmaterial mit standardisierten Kriterien. Darüber hinaus macht der Schengen-Raum mit der Aufhebung der Binnengrenzen in der EU gemeinsame Strategien und Verfahren erforderlich, um einem möglichen Ausbruch von Viren durch transnationale Maßnahmen entgegenzuwirken.

Das TROPICSAFE-Projekt zielt darauf ab, ein standardisiertes Trainingsprotokoll auf europäischer Ebene zu entwickeln, das sich auf die Beförderung von Patienten mit Tropenkrankheiten konzentriert.
Diese Schulung wird auf einer Open Online Learning Platform veröffentlicht. Die Schulung umfasst audiovisuelle Unterstützung, z. B. Videos, Bilder und Grafiken, um die Sicherheitsverfahren, die vom Fachpersonal befolgt werden sollten, besser zu veranschaulichen. Sie wird mit Fragen- und Antworttests zur Bewertung des Schulungsprozesses abgeschlossen und in die Sprachen der Arbeitsgemeinschaft übersetzt.

Die Arbeitsgemeinschaft setzt sich aus fünf Institutionen zusammen, die den Sektor in ihrem Land genau kennen und die Interessen von Arbeitnehmern, Angehörigen der Gesundheitsberufe und Ausbildern vertreten. Unter der Leitung des BNITM sind folgende weitere Einrichtungen an diesem Projekt beteiligt:

HHECO - Der Einfluss von Helicobacter pylori-Koinfektionen auf die Immunregulation und den klinischen Verlauf von HIV-Patienten in Ghana

In letzter Zeit hat das Zusammenspiel zwischen dem Humanen Immundefizienzvirus (HIV) und der Infektion mit Helicobacter pylori die Aufmerksamkeit auf sich gezogen. Eine Reihe von Studien berichtete über günstigere Immun- und klinische Parameter bei HIV-infizierten Personen mit H. pylori. Die inverse Beziehung zwischen der H. pylori-Infektion und der Immunkompetenz des Wirts ist unerwartet, da die meisten chronischen Infektionen bei Patienten mit fortschreitender HIV-Erkrankung häufiger auftreten.

Unser Ziel ist es, die Auswirkungen von H. pylori und anderen relevanten Koinfektionen auf die Immunaktivierung und die klinischen Ergebnisse in einer Kohorte von 1000 HIV-positiven Patienten im Komfo Anokye Teaching Hospital in Kumasi, Ghana, zu untersuchen.


KCS - Kumasi-Kohortenstudie zur Wirksamkeit von ART der ersten und zweiten Linie bei HIV-Patienten in Ghana

Im Jahr 2011 wurden in Ghana 56.000 erwachsene HIV-infizierte Personen (PLHIV) mit antiretroviralen Medikamenten behandelt. Dies entspricht einem Viertel aller Erwachsenen mit HIV in Ghana und einer Abdeckung von 47% bei Menschen mit fortgeschrittener HIV-Infektion.

In einer Kohorte von 1500 HIV-infizierten Patienten mit First-Line-ART im Komfo Anokye Teaching Hospital in Kumasi sollte der Anteil der Behandlungsversager und ihrer Determinaten untersucht werden. Während dieser Studie haben wir eine Kohorte von PLHIV etabliert, die nach virologischem oder immunologischem Therapieversagen auf Second-Line-ART umgestellt wurde. Auch hier wollen wir die Wirksamkeit dieser Second-Line-ART untersuchen, die aufgrund der Tatsache, dass es in Ghana keine Third-Line-Option gibt, von großem Interesse ist.

Das Bernhard-Nocht-Zentrum für Klinische Studien (BNCCT) ist Teil der Sektion Tropenmedizin der I. Medizinischen Klinik des Universitätsklinikum Hamburg-Eppendorf (UKE). Es befindet sich im historischen Gebäude des Bernhard-Nocht-Instituts für Tropenmedizin (BNITM).

Seit 2008 führen wir Impfstudien durch und bieten umfassende Dienstleistungen rund um die Durchführung von klinischen Studien in den Bereichen Reisemedizin und Tropenmedizin.

Zu diesen Leistungen gehören die Leitung von multizentrischen Studien, GCP-konforme randomisierte klinische Studien, Erstellung von studienbezogenen Dokumenten,  Beratungsleistungen einschließlich Studien- und Protokolldesign, sowie Daten- und Sicherheitsüberwachung.
